Job Summary
Coordinate, manage, and execute key care coordination activities for Children’s centralized lead care management program. Utilizing population surveillance tools and data, monitor lead testing care gaps and milestones and referral tracking and care coordination for elevated lead levels. Lead internal and external activities related to the program including patient and family outreach, scheduling, and coordination of transportation, referrals, and connections to internal and community services and programs. Enable capacity building within system partner teams relative to lead care gap closure. Train learners and interns to execute lead care management activities as available. Active partner with community organizations in the lead care continuum. Will have a voice in local, state and federal level activities related to the program as appropriate.
Additional Responsibilities
Conducts multi-modal outreach, scheduling, and responds to children and family's resource needs. Acts as a supportive and respectful liaison between families, providers, and other community agencies as appropriate.
Utilizes population surveillance tools to surface patients needing outreach, care management, care coordination or education.
Provides capacity building with system partner teams relative to lead care gap closure inclusive of staff training and competency building for venous lead care gap closure.
Utilizes the electronic clinical information systems to obtain information and document clinically related activities.
Coordinates with care team members to serve as liaison with community business organizations and governmental agencies to facilitate referral management.
Provides community and stakeholder education and outreach related to the program, including facilitating direct services in programs, arranging program logistics and marketing programs internally and to the community.
Oversees and leads the development of program materials and implementation processes to ensure effective and efficient operations.
Collaborates with Integrated Lead Program leadership to foster strong community partnerships. Partners with community organizations in the lead care continuum inclusive of coordinating and facilitating community-based lead testing events.
Provides patient education and resources to promote improved health outcomes
Must be able to demonstrate knowledge and skills necessary to coordinate care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the coordination needed.
Minimum Job Requirements
Valid Medical Assistant (MA) Certificate
High school graduate or Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED)
Requires 3 years of experience in health promotion, health advocacy, health education, family support or community outreach
Experience working with vulnerable patient populations
Experience integrating with community organizations
Experience engaging external stakeholders to identify mutual goals for the patient/family improved health outcomes
Knowledge / Skills / Abilities

Demonstrated knowledge of psychosocial, economic and cultural determinants of healthy growth and development.
Clinical experience performing direct patient care for pediatric patients and their families.
Skilled with venous specimen collection of the pediatric population.
Skilled at advocating on behalf of patients and families
Strong organizational skills and ability to work independently.
Ability to manage multiple projects within set timeframes.
Ability to manage work across multiple technology platforms and multi-modal outreach systems.
Interest and ability in working collaboratively in a team environment and the ability to work with diverse groups and individuals.
Effective and professional oral and written communication skills including the ability to develop reports and presentations.
Excellent computer skills and well-versed in the Windows environment.
Schedule
This is a hybrid position working both remotely and onsite, with majority of time being remote, but will be onsite weekly at our CMG clinics conducting venous draws. The schedule for this position is generally, 8:00-4:30, however, you must have the ability to work flexible hours to meet the patients' needs with weekend and evening commitments periodically for community based lead testing events.
