What You'll Do

The Case Manager provides trauma-informed, non-medical case management and psychosocial support to individuals living with HIV. Support may include advocacy, health education, resource navigation, care coordination, and emotional support grounded in harm reduction, racial equity, and person-centered care. This role supports clients in navigating complex systems (e.g., healthcare, housing, social services), identifying personal goals, accessing resources, and building community.


This hybrid position offers flexibility based on a candidate’s background and experience. Case Managers are trained service providers who deliver client-centered support and are not required to have an HIV diagnosis. Peer Advocates, by contrast, are individuals with lived experience of HIV and are strongly preferred for their capacity to offer mentorship, model empowerment, and

build trust through shared experience.


Both roles are essential in reducing isolation, strengthening engagement in care, and promoting wellness across a diverse HIV-positive community.



Expectations of your role:
  • Direct Client Support
  • Provide strengths-based, culturally responsive support to BABES members through one-on-one sessions, phone calls, and community-based meetings
  • Conduct intake and needs assessments; create and monitor individualized service plans or peer support goals
  • Assist clients in accessing medical care, transportation, housing, food, benefits, and psychosocial support
  • Act as an appointment partner, when needed, to help clients navigate medical and social service systems
  • Provide HIV-related education, resource referrals, and emotional support
  • Model self-empowerment, healing-centered engagement, and support participants in establishing and maintaining healthy boundaries
  • Documentation and Coordination
  • Track client engagement and services using systems such as Provide, SmartSheet, and other internal tools
  • Maintain client confidentiality in accordance with HIPAA and YWCA policies
  • Support compliance with funder documentation including eligibility, renewals, and case notes
  • Participate in case conferencing and internal team communication
  • Community Building and Outreach
  • Facilitate or support member events, support groups, and outreach activities
  • Build relationships with partner providers and referral agencies
  • Represent BABES Network at community events, educational sessions, and external meetings
  • Professionalism and Peer Leadership
  • Mentor and uplift program participants, drawing from either professional training or lived experience to foster trust, resilience, and growth
  • Participate in ongoing trainings to strengthen knowledge in areas such as HIV care, trauma-informed practices, de-escalation, peer support, and community-based service delivery


Must have's to be successful:
  • BA in social services preferred, equivalent experience considered.
  • Min. 3 years related experience working with persons with disabilities, specifically in the mental health and/or chemical dependency field.
  • Knowledge of the Section 8 program and housing resources in King County and Snohomish County, Landlord/Tenant Laws and Fair Housing Regulations.Experience in housing homeless people with barriers is required.
  • Knowledge or experience working in housing and urban development (HUD) regulations.
  • Case Management experience a plus.
  • Must be able to travel independently between multiple work sites within a day.


Physical Requirements
  • The physical demands described here are representative of those that must be met to successfully perform the essential functions of this role.
  • Frequently uses speech, hearing, and vision to communicate with clients, staff, and community partners
  • Frequently uses hands and wrists for typing, data entry, and handling materials or supplies
  • Frequently sits for extended periods during desk work, meetings, and virtual sessions
  • Occasionally moves items up to 20 pounds
  • Regular local travel required for community events, outreach, or off-site meetings
  • Indoor office and community-based environments
  • All positions at YWCA Seattle | King | Snohomish are exposed to clients who have experienced trauma, including but not limited to: violence, homelessness, systemic discrimination, and financial hardship. As a result, staff may be at risk of secondary trauma. Employees are encouraged to practice self-care and seek support as needed.


$1 - $1 an hour
Hourly Rate: $26.50
Hours: 40 hours per week
Excellent benefits package including medical insurance, retirement plan, plus generous vacation, holiday, and sick leave plans
At the time of hire, employees may enroll voluntarily in the Fidelity 403b Plan
After two years of employment, employees are eligible to participate in the YWCA Retirement Fund
