Nurse Practitioner (Hybrid Remote)

Strive Health focuses on transforming kidney care by providing coordinated, patient-centered care through an interdisciplinary team. They seek a skilled Nurse Practitioner to deliver advanced nursing care for CKD and ESKD patients, combining remote and in-person visits.

Skills

  • Proficiency in clinical assessment and management of chronic kidney disease and related co-morbidities.
  • Ability to collaborate effectively with multi-disciplinary teams and external providers.
  • Strong communication and patient relationship building skills.
  • Experience with patient care delivery in varied settings including telehealth.
  • Competency in data management related to clinical, financial, and patient satisfaction outcomes.

Responsibilities

  • Manage care for CKD and ESKD patients utilizing an interdisciplinary clinical team.
  • Perform patient histories, physical exams, diagnostic test interpretation, and individualized care planning.
  • Collaborate with external providers to align treatment plans and clinical goals.
  • Deliver care across multiple settings including patient homes, clinics, and virtual visits.
  • Advocate for patients and support transitions to renal replacement therapy or advanced care planning.
  • Participate in clinical rounds and interdisciplinary team meetings.
  • Maintain current board certification and comply with continuing education requirements.
  • Adhere to organizational documentation policies and standards.

Education

  • Master’s degree in Nursing or similar field required.
  • National Board Certification required.
  • Current state-specific nursing licensure required.
