Responsibilities

- Perform patient histories, physical exams, diagnostic test interpretation, and individualized care planning.
- Collaborate with external providers to align treatment plans and clinical goals.
- Deliver care across multiple settings including patient homes, clinics, and virtual visits.
- Advocate for patients and support transitions to renal replacement therapy or advanced care planning.
- Participate in clinical rounds and interdisciplinary team meetings.
- Maintain current board certification and comply with continuing education requirements.
- Adhere to organizational documentation policies and standards.
Education
- Master’s degree in Nursing or similar field required.
- National Board Certification required.
- Current state-specific nursing licensure required.
