Job Description

Division/Program Summary:
The New York City Department of Health and Mental Hygiene (DOHMH) is expanding its commitment to reducing health disparities and improving outcomes for families through the New Family Home Visits (NFHV) Initiative. This program delivers comprehensive, evidence-based home visiting services designed to improve maternal and child health outcomes in neighborhoods disproportionately affected by racial, ethnic, and socioeconomic inequities. As DOHMH scales NFHV to reach more families, the Public Health Advisor II role is essential to the successful delivery of services.

Position Summary:
Reporting to the Supervising Public Health Advisor, the Public Health Advisor II will provide frontline services to expectant and new mothers. Through home visits and ongoing engagement, the advisor will deliver targeted health education, conduct assessments, and coordinate access to care and resources. This role plays a critical part in addressing preventable maternal and infant health issues and promoting equity across New York City communities. This position ensures that families receive consistent, high-quality health education, screenings, and referrals in the comfort of their homes supporting both immediate and long-term health outcomes for mothers and infants.

Job Duties and Responsibilities:
- Conduct home visits to eligible mothers, delivering prenatal and post-partum education, health assessments, and referrals.
- Provide maternal and infant health education, covering topics such as breastfeeding, safe sleep, mental health, and chronic illness.
- Assess home environments for health hazards like lead, making necessary referrals for follow-up.
- Collect, monitor, and track case data, ensuring accurate documentation.
- Connect families to internal program specialists (e.g., lactation consultants, social workers) and external community-based services.
- Follow up on external referrals with post-visit phone calls.
- Engage with community providers to identify and recruit eligible families.
- Promote the program at community events, conduct neighborhood health assessments, and support senior management with additional tasks.
- Represent the NFHV program at community events, assist with neighborhood-level health assessments, and support program leadership with additional assignments as needed.
- Participate in marketing program through community events, conduct neighborhood health assessments and/or surveys, and assist senior management in other assignments.

PUBLIC HEALTH ADVISER - 51191

Qualifications

1. A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education, or in health, social or biological sciences; or
2. A baccalaureate degree from an accredited college, and six months of full-time satisfactory experience in a health promotion or disease intervention/prevention program, performing one or more of the following: interviewing, conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data; or
3. A four-year high school diploma or its educational equivalent, and four years of full-time satisfactory experience as described in "2" above; or
4. Education and/or experience equivalent to "1", "2" or "3" above. Undergraduate college credit can be substituted for experience on the basis of 30 semester credits from an accredited college for one year of full-time experience. However, all candidates must have a four-year high school diploma or its educational equivalent, and either twelve semester credits as described in "1" above or six months of experience as described in "2" above.
Additional Requirements
A. To be assigned to Assignment Level II, candidates must have, in addition to meeting the minimum qualification requirements listed above, at least one year of experience as a Public Health Adviser, Assignment Level I, or at least one additional year of experience as described in Qualification Requirement "2" above.
