This Position is Contingent Upon Contract Award.

Under the direction of the National Medical Director and in collaboration with the Office of the Chief Medical Officer (OCMO) of U.S. Customs and Border Protection (CBP), Regional Physician Supervisors deliver healthcare services to persons in custody in accordance with the polices and protocols listed in the Statement of Work (SOW) and Department of Homeland Security (DHS) directives for the delivery of healthcare services to persons in custody. Regional Physician Supervisors share the administrative functions that directly impact medical services and collaborate with the management team in overall planning and execution of program directives.  

Regional Physician Supervisors work to assure delivery of quality healthcare services in accordance with the SOW for the medical services contract within medical units at U.S. Customs and Border Protection sites including, Office of Field Operations (OFO) ports of entry, U.S. Border Patrol Border (USBP) Stations. Performs the clinical responsibilities as agreed upon by the National Program Director.  

Contracted medical staff will be working alongside uniformed Customs and Border Protection (CBP) Agents/Officers in secured federal facilities along the southwest border of the United States. These secured federal facilities are staffed and monitored by CBP personnel and/or contracted security staff 24 hours a day, 7 days a week to assure a safe working environment to include the co-located medical unit. Contracted medical staff work together with CBP personnel to provide health care services for persons of all ages, to include infants, children, adults, and pregnant females. 

MISSION  

CBPs Office of the Chief Medical Officer (OCMO) employs a trauma informed care model to provide frontline medical care for persons in CBP custody in accordance with Enhanced Medical Support Efforts CBP Directive (2210-004). This model provides a team of medical professionals comprised of physicians, advanced practice providers (e.g., physicians assistants, nurse practitioners) and support staff (e.g., Licensed Practical/Vocational Nurses, Paramedics, EMTs, Certified Medical/Nursing Assistants). These contracted medical teams provide basic assessment and medical treatment for persons in CBP custody to include emergency care as needed. 

Duties and Responsibilities

• Serves as the sector/region medical authority for the medical services contract and medical units at for the U.S. Customs and Border Protection sites including, Office of Field Operations (OFO) ports of entry, and/or U.S. Border Patrol Border (USBP) Stations.  
• Provides medical direction and necessary guidance/oversight as required by state licensure for appropriate advanced practice provider supervision, oversight, consultation, coordination, and direction for all medical services, to include mental health and pediatric care in the absence of a behavioral health advisor or pediatric advisor.  
• Clinical supervisory authority over sector physicians, mid-level providers, all clinical staff, all medical unit functions, laboratory, and pharmacy.  
• Provides on-call coverage for assigned medical units/sector 24 hours a day/7 day a week, however, call coverage may rotate based upon staffing.  
• Responsible for developing and coordinating all aspects of healthcare delivery at his/her assigned facilities.  
• May provide direct care to patients in accordance with current accepted practice, licensure, certification, credentialing and or granted privileges.  
• Maintains non-involvement in the security aspect of patient care and custody.  
• Participate in disease surveillance activities and public health and epidemiology studies/efforts, as needed.  
• Provides age-appropriate screening, treatment, and patient instruction pertinent to the discipline and facility setting for acute and chronic conditions for all ages.  
• Serves as the laboratory director in sites where CLIA waived testing is performed, providing oversight of appropriate diagnostic tests and services, to include monitoring and meeting CLIA requirements.  
• Acts as a reviewer for all outside referrals/consults to specialty services, to include hospital or emergency care that exceed services offered at the medical unit. Audits referrals and post-referral follow-up, as needed, as part of the medical quality management (MQM) program.  
• Provides oversight for any patients requiring enhanced medical monitoring.  
• May be expected to respond to and or coordinate response to medical emergencies and medical sentinel events.  
• Seeks guidance from the Medical Director, Pediatric Advisor, Behavioral Health Advisor, or other leadership as appropriate.  
• Serves on the national and local MQM committees and oversees root cause analyses and quality improvement projects/initiatives in collaboration with the national and local MQM committee.  
• Attends/Leads general staff meetings at assigned medical units and leads or oversees MQM initiatives and reviews, working in close collaboration with the local physician and Patient Safety Risk Manager, as applicable.  
• Performs record keeping functions in accordance with program policies and position.  
• Maintains functional proficiency and ease of use utilizing electronic health records as required by, and appropriate to, position.  
• Completes all initial, annual, and ad hoc training or competencies as required/assigned.  
• Maintains patient confidentiality, and confidentiality of medical records, in compliance with the Privacy Act and HIPAA regulations in all work activities as applicable.  
• Adheres to, and maintains awareness of DHS and CBP Policies, Procedures, Directives, and Operational Memoranda.  
• Leads, attends, and participates in general/medical staff meetings at the local and national level.  
• Performs chart reviews as part of MQM, focused professional practice evaluation (FPPE), on-going professional practice evaluation (OPPE), annual performance evaluation, medical sentinel event reviews, quality assurance and perform improvement initiatives.  
• Completes summaries of all completed chart reviews, identifying trends, performance concerns, best practices. Some items may be delegated if a local physician is assigned at a specific facility, for those assigned personnel to that facility.  
• Management and oversight of the Peer review program for the Physicians and APPs assigned as part of OPPE and completion of an annual evaluation process for the assigned Physicians and APPs.  
• Performs collaborative mid-term review of performance with assigned Physicians and APPs as part of an annual performance evaluation process, addressing any clinical practice concerns if identified.  
• Portions of the APP peer and chart review process, FPPE, and OPPE may be delegated, if a physician is assigned to the local facility.  
• Provides emergency care to employees or visitors for life, death, loss of sight/limb situations until Emergency Medical Services Arrives.

BASIC REQUIREMENTS  

Education/Training 

• Degree: Doctor of Medicine, Doctor of Osteopathic Medicine or equivalent from a school in the United States or Canada. The degree must have been accredited by the Council on Medical Education of the American Medical Association, Association of American Medical Colleges, Liaison Committee on Medical Education, Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.  
• Degree from Foreign Medical School: A Doctor of Medicine or equivalent degree from a foreign medical school must provide education and medical knowledge equivalent to accredited schools in the United States. Evidence of equivalency to accredited schools in the United States is demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates, a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country, or successful completion of the U.S. Medical Licensing Examination.  

Licensure/Certification  

• Candidates must have a current, active, valid, and unencumbered license, registration, or certification as a Doctor of Medicine (M.D.) or as a Doctor of Osteopathy (D.O.) issued by any United States jurisdiction. Licensure must be maintained current, and any required education or trainings to maintain licensure must be completed within the required timeline of the licensing authority. A temporary license will not be accepted.  
• Candidate must not have had any professional licensure or certification withheld, suspended, revoked, or denied, and the candidate must not have surrendered or allowed a professional license or certification to have expired or lapsed because of an investigation or disciplinary action.  
• Candidate must not have been convicted of, pled guilty to, pled nolo contender to, or received probation before judgement for any crime other than a minor traffic violation, the record of which has not been expunged.  
• Current unexpired Basic Life Support (BLS) Certification must be maintained from one of the following approved organizations:  
  • American Heart Association (AHA) BLS for health care providers  
  • American Heart Association Advanced Cardiac Life Support (ACLS), if applicable  
  • American Red Cross CPR/BLS for the professional rescuer  
  • American Safety and Health Institute (ASHI) CPR and AED for healthcare providers and professional responders  
    • Course components must include Adult, Pediatric, and Infant CPR, Foreign Body Airway Obstruction, and Automatic Defibrillator (AED) proficiency  
    • Applicant/ Employee must be certified upon hire and may also be a certified instructor  
    • BLS must be renewed prior to expiration date.  
• Board certified/board eligible in a primary care field, to include Family Practice, Internal Medicine, or Emergency Medicine by the American Board of Medical Specialties (ABMS) or American Osteopathic Association (AOA) approved board. Other relevant board certification / eligibility will be considered by OCMO upon request. 

Experience  

• Subsequent to obtaining a Doctor of Medicine or Doctor of Osteopathic Medicine degree, a candidate is required to have successfully completed a medical residency program in a hospital or an institution accredited for such training. For purposes of this standard, graduate training programs include only those internship, residency, and fellowship programs that are approved by accrediting bodies recognized within the United States or Canada.  
• Residency trained and board certified is required.  
• 5 years' experience as attending or independently practicing physician in a direct patient care setting.  
• Experience in detention/correctional healthcare, humanitarian care, ambulatory care, or a pediatric setting encouraged, but not required. 

Knowledge, Skills, and Abilities  

• Expert knowledge of the current standards of practice in clinical medicine.  
• Ability to serve proficiently as a clinical expert, providing technical assistance and consultation to clinic medical staff.  
• Ability and proficiency to properly use equipment, materials and supplies in simple diagnostic and treatment procedures.  
• Ability to provide input for congressional inquiries and to effectively communicate with Federal Agencies and other organizations as indicated.  
• Mastery of managerial and administrative practices, to include expertise in routine management of ambulatory clinic functioning and responsibilities.  
• High degree of independence, initiative, clinical judgment, and follow-through on a wide range of sensitive, complex, and clinical issues.  
• Teaching experience acknowledging a level of expertise and capacity for developing and accessing clinical educational programs.  
• Knowledge of issues confronting the healthcare delivery system, including specific problems and concerns of special population groups.  
• Knowledge of program policies and guidelines, and operating procedures relating to healthcare delivery systems.  
• Ability to work in a multi-cultural and multi-lingual environment.  
• Ability to utilize telephonic interpreter/translation services to complete assigned duties if not fluent in a language the patient understands.  
• Ability to adapt to sudden changes in schedules and flexibility in work requirements.  
• Ability to communicate proficiently in English (verbal and written) in order to develop positive rapport with patients, co-workers and other stakeholders.  
• Ability to establish and maintain positive working relationships in a multidisciplinary environment.  
• Ability to navigate in an electronic work environment including electronic health records, web-based training and communications. 
• Moderate proficiency in, common word processing, presentation and spreadsheet software programs.  
• Knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII).  
• Ability to initiate and maintain compliance with DHS and CBP policies and any overriding federal standards/policies, or accreditation standards that may apply.  
• Ability to develop and maintain compliance with standard operating procedures and protocols.  
• Ability to perform record keeping functions in accordance with program policies and the system of records management.  
• Ability to use the electronic medical record and maintain an “active status” for use and complete all required trainings.  
• Ability to utilize the electronic medical records system for all documentation, to include, but not limited to all patient care, monitoring, medication administration/observation documentation, and input of any paper records as applicable.  
• Ability to assure a safe and clean working environment and assists in the disposal of biologically hazardous waste on a regular basis.  
• Ability to oversee laboratory services and has awareness and ability to perform quality control of laboratory reagents and perform CLIA waived testing as needed.  
• Ability to completes all annual training or additional training as directed.  
• Ability to travel to each assigned medical unit quarterly and be able to work at an assigned medical unit daily to provide oversight as required.  
• Ability to assist with patient care in time of staff shortage or surge.  
• Ability to serve at or provide patient care at alternate medical units within assigned sector/region on an as needed basis as required.  
• Ability to be available telephonically and/or to report to the medical unit during non-core hours. 

Supervisory Controls 

• Works under the administrative direction of the Medical Director or in his/her absence the Deputy Medical Director.  
• Works under the clinical supervision and/or direction of the Deputy Medical Director or in his/her absence the Medical Director. 
• In this position, the employee will serve as a contractor to the federal government and does not have any federal authority or rights. 

Physical Demands  

• Able to walk, stand, bend, or sit for extended periods.  
• Able to reach to retrieve documents or supplies.  
• Able to carry materials and/or file boxes up to 30 pounds on an infrequent basis or assist patients into/out of a wheelchair.  
• Must be capable of standing on hard surfaces (cement floors) for long periods of time.  
• Must perform the duties in a stressful and often austere environment without physical limitation.  
• Must be able to respond to a medical emergency and perform BLS/CPR as needed. 

Other Special Qualifications

• US Citizen or Permanent Resident Card is required.    
• Ability to obtain and maintain the U.S. Government security clearance. Applicants with current clearances are preferred.  
• Must be able to read, write and speak English to effectively communicate. 
• Proficiency in both English and Spanish is preferred. 

Pay range is based on several factors and may vary in addition to a full range of medical, financial, and/or other benefits. Final salary and offer will be determined by the applicant’s background, experience, skills, internal equity, and alignment with geographical market data.
