Case Manager – Full-Time Position
Location: Alameda, CA
Schedule: Weekdays, 9:00 AM – 5:00 PM
Salary Range: $100,000 – $120,000 per year
Job Summary:
We’re hiring a Case Manager to provide in-office client support. The person in this role will serve as a reliable resource for individuals who need help accessing services, navigating support programs, and overcoming obstacles to stability. You'll oversee each case from start to resolution, offering both structure and encouragement along the way.
Duties:
- Create individualized care plans and timelines
- Arrange and confirm appointments with providers
- Maintain case notes and documentation
- Communicate with families, professionals, and service teams
- Track goals and reassess as needed
- Bachelor’s in Social Work, Psychology, Nursing, or related field
- 2+ years in a case management or similar support role
- Clear communicator with strong time-management skills
- Knowledge of local community networks and providers
- Licensure (LCSW, LMSW, RN) preferred but not required
