Responsibilities
- Build empathetic relationships with patients and families.
- Conduct patient education regarding diagnosis, prognosis, and treatment options.
- Evaluate patients' needs through advance care planning and goals of care discussions.
- Develop collaborative care plans addressing symptom palliation and clinical needs.
- Identify and address social determinants of health impacting patients.
- Participate in interdisciplinary team meetings and patient case conferences.
- Coordinate care across providers including palliative and hospice services.
- Maintain accurate patient care records and communicate plans across settings.
- Educate internal and external partners about advance care planning and referral processes.
- Represent Evergreen Nephrology with professionalism to patients and the community.
Education
- Current RN License required.
- Bachelor of Science in Nursing (BSN) preferred.
